- Print out the forms listed below
and fill them out. Bring them with
you, or fax them prior to your
appointment.
- Bring your insurance card to
each appointment.
- Bring all prescription and
over-the-counter medication that you
are currently taking.
- A list of all questions and/or
concerns that you may have.
Please do not bring children under
the age of nine to your appointment.
This will ensure your full attention
with the doctor.
