Please bring the following items with you to your appointment:
  • Print out the forms listed below and fill them out. Bring them with you, or fax them prior to your appointment.
  • Bring your insurance card to each appointment.
  • Bring all prescription and over-the-counter medication that you are currently taking.
  • A list of all questions and/or concerns that you may have.

Please do not bring children under the age of nine to your appointment. This will ensure your full attention with the doctor.

Preparation and communication are key factors in enhancing health outcomes.
